City Care Partners has applied to participate in the CMS LEAD Model beginning January 1, 2027, subject to CMS approval and execution of participation agreements.
Your Medicare benefits do not change. You keep your choice of doctor.

Being part of an ACO doesn't change your Medicare benefits or your right to see any doctor who accepts Medicare. We work quietly in the background to coordinate your care — you keep all the freedom you have today.
